Use of Site
Doctize operates the website www.doctize.com and other related extensions and mobile applications with links to this website. We provide doctors with which the website visitor obtains services and medical care. By using the site you agree with this, and all other conditions to be specified on the site. If you do not agree, you may not use the Site and its services.

Services rendered
Doctors who provide the services through Doctize are BIG registered doctors and therefore responsible for the care they provide to you. Doctize and/or third parties who recommend the services are not liable for the professional advice and treatment you get through Doctize. Doctize monitors its doctors by using a rating system and periodic meetings with the CMO (chief medical officer). Malfunctioning doctors will always directly be pulled away from the active service.

Privacy
The information you entrusted to the doctor during the consultation is legally confidential. The information you provide online when making the appointment shall also be handled very carefully. We do our utmost to ensure that this personal information is secure. Also see our privacy statement.

Users
When you register on the site, you are required to provide us with your first name, last name, phone number, email address, password, and possibly other important notes. If you want to create an account, you must have the legal age to form a binding contract. If you do not have that age, you cannot register yourself and not use our services. You agree that all information (including any uploads) when registering and making the appointment, is accurate, up to date and complete. The account is not transferable and the corresponding password is not to be shared with anyone. It is not allowed to create multiple accounts in the same name. You are responsible for maintaining the confidentiality of your account, your password and for all activities that occur within that account. Releasing your account information to third parties is prohibited. Also the use of the account of a third party is prohibited.
Doctize reserves the right to take the measures it deems necessary or reasonable, with regard to the security of the site and your account information. Doctize cannot be held responsible for any liability or damage arising from use of the site and/or your account.

Doctize and Children
The Doctize services are also available for children. However, the account of someone aged under 18 should be managed by a parent or legal guardian. If you create an account for those under 18 you are fully responsible for him or her and you have to abide by all conditions.

Acces rights
We hereby grant you a limited, non-exclusive, non-transferable right to access the website and use of the services. The services are exclusively for personal non-commercial use and only permitted in these terms and any other individual agreements with us.
You agree that you will never try:

• To act in name of a different person or entity or otherwise, or to act uncorrectly as a relationship of a person or entity.
• Utilizing the site or services with the aim to violate law.
• Reverse to engineer, disassemble, decompile, or duplicate the technology/software/scripts from the site or services.
• To distribute viruses or other malicious code through using the site, the services or otherwise.

Furthermore, you agree to refrain yourself from abusive language and behavior that could be considered inappropriate or unlawful behavior during use of the services provided by Doctize. Doctize is not responsible for any interactions of doctors and user conducted outside of the site. We advise you not to use the services on public computers. We advise you not to save the password for your account through your web browser or other software.

Fees and purchase
You agree to pay all fees or charges that were agreed upon at the time of booking the services of Doctize.
By carrying out the payment you agree to all terms of Doctize. The amount appropriate to the services will be cashed in advance and is for your own account. Doctize is not obliged to give additional notice or ask for additional consent. In some cases, the health insurance might cover the costs. Doctize does not guarantee that you will receive such a reimbursement and it is up to you to find out. Doctize reserves the right to modify the rate structure at all times.

Cancellations
You understand and agree that you are responsible to comply with the agreements made. Agreements cannot be canceled. If you are unable to fulfill the scheduled appointment, you can change this without extra costs up to 24 hours to a different time. Changing an appointment should always take place in the same week as the original appointment. If you are not present at the agreed time of the consultation you accept that the costs cannot be reimbursed. In the event that an appointment is made within less than 24 hours it cannot be changed.
Doctize reserves the right to modify appointments at all times. Such change is always in agreement with you. If we cannot reach an agreement you can claim a refund.

Right to change
Elements of these terms and conditions can be changed, added or removed at all times. In such case, changes in the conditions do not apply to already existing agreements.

Liability statement
You explicitly agree that use of the site and/or services is at your own risk. Both the site and the services are provided as available at the time of use. Doctize disclaims all warranties with regard to availability of the services. You acknowledge and accept that Doctize itself does not provide medical advice, diagnosis or treatment. Doctize is strictly a technical platform and provides the infrastructure for linking users with doctors. You acknowledge that the doctors themselves are responsible for providing medical advice and any treatments. The doctors determine whether additional consultation with the Chief Medical Officer is deemed necessary.

Disclaimer
You indemnify Doctize and all its employees from all liabilities, actions, demands, claims arising from your violation of these terms, as well as doctors of Doctize.
